We are a self-pay only practice.  We are happy to help you recieve your reimbursement for out-of-network benefits with your insurance carrier.  Please check with your insurance provider to see what your out-of-network benefits include.  If you have questions, please do not hesitate to call our office.

Please ask to have your patient forms emailed to you, so you can complete them and bring with you to your first appointment.  If you do not complete the forms prior to the first appointment, please arrive 15 minutes early to complete your paperwork.
